BISD is partnering with Blanco Pharmacy & Wellness to provide flu shots for students who are at least 9-years-old or older as well as all BISD staff! Consent forms can be found on the BISD Website. Look for the form entitled Screening Checklist for Contraindications to Vaccines for Adults. Flu shot consent forms will also be available in the front office at each BISD campus. Completed consent forms must be returned by Tuesday, October 15th.

Please attach a copy of your insurance or Medicaid card with the completed consent form and return them to the campus where your child will receive their shot. You may also email the completed paperwork to angela.kuzins@blancoisd.org  If you do not have medical insurance or Medicaid, flu shots will be available for $35 and must be prepaid.
