Shot Record Information

Attention Parents of Incoming 7th graders:

Did you know that you can view your child’s shot record on the BISD Parent Portal? Log in to your portal and click on your child’s immunizations tab.

• Your 7th grader will need 2 additional immunizations before school starts on August 16th.

• If your child has not received Tdap or MCV4 (meningitis), they will not be allowed to begin classes on the first day of school.

• Please return the updated shot record to Nurse Johnson BEFORE the 1st day of school (August 16th) or they will not receive their class schedule.

• Do not wait until August to get these shots…Dr.’s offices, pharmacies, and Health Department locations may run out.

If your child has an Exemption Affidavit that will expire prior to August 16th, a notarized original must be provided before the first day of school.
